Child Medical Statement
All preschool children must have an updated medical statement on file at the start of each school year. This form is completed and signed by your child’s medical doctor and includes a statement that they in suitable condition to participate in group care. It also requests your child’s current up-to-date immunizations. This information must be current and within the last year. Please note: Immunizations of all students must be in compliance with the Ohio Revised Code. Failure to provide this document with written evidence of immunization is cause for exclusion from school per the Ohio Department of Health.

Procedural Safeguards Notice
This document is fo parents of children who receive or may be eligible for special education services. Parent of children with disabilities have rights under The Individuals with Disabilities Act (IDEA 2014) and ensures parents right to participate in their children’s education. A Guide to Parents Rights in Special Education ~ Special Education Procedural Safeguards Notice will provide parents with an overview of this law will assist in understanding parents rights and responsibilities in the special education process. The school district must give you a full copy of the Notice of Procedural Safeguards.
